Your PHI includes information about your mental health condition, treatment provided, and payment for services. This includes information created or received by Tracy Parsons, ALC that identifies you and relates to your past, present, or future physical or mental health condition.

Treatment: Your PHI may be used to provide, coordinate, or manage your mental health care and related services.

Payment: Your PHI may be used to obtain payment for your healthcare services, including insurance billing and collection activities.

Healthcare Operations: Your PHI may be used for quality assessment, professional review, and business planning activities.

When Authorization is Required

Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and disclosures that constitute a sale of PHI require your written authorization. You may revoke authorization at any time by providing written notice, except to the extent that action has already been taken.

Your Rights Regarding Your PHI

Right to Inspect and Copy

You have the right to inspect and copy your PHI, except psychotherapy notes. To access your records, submit a written request. A fee may be charged for copying costs.

Right to Amend

You have the right to request that we amend your PHI if you feel it is incorrect or incomplete. We may deny your request for certain legal reasons.

Right to Request Restrictions

You have the right to request restrictions on how your PHI is used or disclosed. We are not required to agree to your request but will consider each request carefully.

Right to Confidential Communications

You have the right to request that we communicate with you about your PHI in a certain way or at a certain location to maintain your privacy.

Right to a Breach Notification

You have the right to be notified if there is a breach of your unsecured PHI.

Disclosures Without Authorization

In certain situations, we may be required or permitted to disclose your PHI without your authorization:

Emergency situations to prevent serious harm to you or others
Child abuse or neglect as required by Alabama law
Adult abuse when mandated by state reporting requirements
Court orders or legal proceedings when required by law

Filing a Complaint

If you believe your privacy rights have been violated, you may file a complaint with our practice or with the Secretary of the Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Changes to This Notice

We reserve the right to change this notice and make the new provisions effective for all PHI we maintain. If changes are made, we will provide you with a revised notice at your next appointment or upon request.
